New option for Rural Generalists in Central NSW

Western NSW Local Health District, together with its partners, continues to review and innovate to attract Doctors to our District to deliver primary care and acute medical services in our remote towns.

One such community, fondly referred to as the 4T’s is a pilot region for a new innovation in primary care. The 4T’s are the towns of Tottenham, Tullamore, Trundle and Trangie in Central NSW, small neighbouring towns with regions of less than 1500 people each, situated approx. 30 minutes from the next by car.

The FourT’s model

4T’s is a comprehensive primary care service across 4T’s communities with Rural Generalists working as a collaboration team to provide high-quality health care. One of 5 pilot sites across Australia, the 4T’s fully networked single employer model of Rural Generalism is supported by the Collaborative Care Program, NSW Rural Doctors Network and Western NSW Primary Health Network, Local Councils and community groups.

The model recognises clinicians, by and large, want to focus on quality medicine, and many, particularly at the beginning of their Rural Generalism career, may not want to shoulder the risk and additional workload of setting up and managing their own practice.  4T’s also addresses the level of comfort a GPs has with acute and inpatient care.


Being an employee has its advantages

As an employee of NSW Health, superannuation, sick leave, long service leave and annual leave is accrued.  Salary packaging and novated leasing as well as an extensive package of incentives and training options are available to Doctors joining the 4t’s network.   Salary is not dependent on billings.

The 4T’s model also subscribes to the notion of ‘easy in, gracious out’ and the network fully supports the development and progression of your career – even if it means outside of our District.

You are never on your own

As a member of the 4T’s network, the infrastructure that underpins the delivery of services is outstanding. Primary care services are co-located with the town Multipurpose Service, linking seamlessly through a single-employer model with the acute services in these towns.

Doctors receive support from a dedicated team of administrative, nursing and practice staff coordinated by the 4T’s Medical Centre Manager.  24/7 Telehealth I used to access a broader network of peers for advice and clinical input when required for emergency presentations and for access to specialist care.

4Ts Doctors will also utilise telehealth to support their colleagues across communities whether to assist in workload pressures, leave relief or just to discuss difficult cases.  Out of hours, category 3-5 presentation are managed by the Virtual Rural Generalist team via Telehealth, ensuring you get a good night’s sleep and appropriate work/life balance.

In short in the 4T’s network you are never on your own.

The central location of 4T’s means that you are no more than 90 minutes drive away from direct flights to Sydney, Melbourne, Brisbane and Canberra with easy accessibility by road to the regional cities of Dubbo and Orange.

Your communities


Tottenham (Pop 453 in 2016) is a recognised as the closest township to the geographical centre of NSW. It is home to a very close knit community, known for their volunteering power and support of one another. Initially a mining town, Tottenham now specialises in the production of wheat and other grains, alongside the production of prime lamb, wool and beef.

Located in central NSW, Tullamore (pop 424 in 2016), has a big mining history. Gold mining and took place in the nearby Fifield area followed by Magnesite later mined for many years. Today, Agriculture is the main industry for the Tullamore area.
